     Relationship with Students and Colleagues
     Outside Activities 
     What makes this teacher unique?
     Additional information (honors, publications, etc.)

    Math level
    Participation and placement in competitions such as Mathletes, 
       Mathcounts, SCMTA Contest, NYSML/ARML, Math Fair, etc.
    GPA
    Other math-related activities
    Distinctions and achievements
